We are excited to provide you with a FREE, curbside pick up meal for your entire family each month. You will pick this meal up from the restaurant. This order form is for Wednesday, January 27th. Your pick up time will be between 4:30 PM - 6:00 PM (you will receive your window pick up time closer to the date). 

Please complete this order form by January 14th. 

When ordering, please choose only ONE restaurant.

PLEASE NOTE EACH RESTAURANT CAN ONLY MAKE A CERTAIN AMOUNT OF MEALS TOTAL. IF THE MAXIMUM AMOUNT IS REACHED AT THE RESTAURANT, THAT RESTAURANT WILL BE REMOVED FROM THE CHOICES BELOW:

5. Sangamo Club (227 E ADAMS STREET, SPRINGFIELD, IL 62701) - Baked Chicken, Rosemary Roasted Potatoes, and Lemon and Garlic Swiss Chard
